mmc: tmio, renesas_sdhi: set mmc_host_ops hooks directly

Drivers can set any mmc_host_ops hooks between tmio_mmc_host_alloc()
and tmio_mmc_host_probe().  Remove duplicated hooks in tmio_mmc_host.
