A null dereference or oops exception will eventually occur when qla1280.c

think its clear from the code that the intention here is sg_dma_len(s) not

After commit 1bad6c4a57 ("scsi: zero per-cmd private driver data for each

In combination with commit 464a00c9e0 ("scsi: core: Kill DRIVER_SENSE"),
after virtio_scsi performs a capacity expansion, the first request will
return a unit attention to indicate that the capacity has changed. And then
the original command is retried. As driver-private command data was not

Fail I/Os instead of retry to prevent user space processes from being
blocked on the I/O completion for several minutes.
Retrying I/Os during "depopulation in progress" or "depopulation restore in
progress" results in a continuous retry loop until the depopulation
completes or until the I/O retry loop is aborted due to a timeout by the
scsi_cmd_runtime_exceeced().
Depopulation is slow and can take 24+ hours to complete on 20+ TB HDDs.
Most I/Os in the depopulation retry loop end up taking several minutes
before returning the failure to user space.

The default handling of the NOT READY sense key is to wait for the device
to become ready. The "wait" is assumed to be relatively short. However
there is a sub-class of NOT READY that have the "... in progress" phrase in
their additional sense code and these can take much longer. Following on
from commit 505aa4b6a8 ("scsi: sd: Defer spinning up drive while SANITIZE
is in progress") we now have element depopulation and restoration that can
take a long time. For example, over 24 hours for a 20 TB, 7200 rpm hard
disk to depopulate 1 of its 20 elements.
Add handling of ASC/ASCQ: 0x4,0x24 (depopulation in progress)
and ASC/ASCQ: 0x4,0x25 (depopulation restoration in progress)
to sd.c . The scsi_lib.c has incomplete handling of these
two messages, so complete it.

In StorVSC, payload->range.len is used to indicate if this SCSI command
carries payload. This data is allocated as part of the private driver data
by the upper layer and may get passed to lower driver uninitialized.
For example, the SCSI error handling mid layer may send TEST_UNIT_READY or
REQUEST_SENSE while reusing the buffer from a failed command. The private
data section may have stale data from the previous command.
If the SCSI command doesn't carry payload, the driver may use this value as
is for communicating with host, resulting in possible corruption.
Fix this by always initializing this value.

Commit 9604eea5bd ("scsi: st: Add third party poweron reset handling") in
v6.6 added new code to handle the Power On/Reset Unit Attention (POR UA)
sense data. This was in addition to the existing method. When this Unit
Attention is received, the driver blocks attempts to read, write and some
other operations because the reset may have rewinded the tape. Because of
the added code, also the initial POR UA resulted in blocking operations,
including those that are used to set the driver options after the device is
recognized. Also, reading and writing are refused, whereas they succeeded
before this commit.
Add code to not set pos_unknown to block operations if the POR UA is
received from the first test_ready() call after the st device has been
created. This restores the behavior before v6.6.

Currently, if CONFIG_SCSI_HISI_SAS_DEBUGFS_DEFAULT_ENABLE is enabled, the
memory space used by DFX is allocated during device initialization, which
occupies a large number of memory resources. The memory usage before and
after the driver is loaded is as follows:
Memory usage before the driver is loaded:
$ free -m
total used free shared buff/cache available
Mem: 867352 2578 864037 11 735 861681
Swap: 4095 0 4095
Memory usage after the driver which include 4 HBAs is loaded:
$ insmod hisi_sas_v3_hw.ko
$ free -m
total used free shared buff/cache available
Mem: 867352 4760 861848 11 743 859495
Swap: 4095 0 4095
The driver with 4 HBAs connected will allocate about 110 MB of memory
without enabling debugfs.
Therefore, to avoid wasting memory resources, DFX memory is allocated
during dump triggering. The dump may fail due to memory allocation
failure. After this change, each dump costs about 10 MB of memory, and each
dump lasts about 100 ms.

During initialization, the driver allocates wq->pring in lpfc_wq_create

called.
There is a possible memory leak in lpfc_sli_brdrestart_s4() (restart)
and lpfc_pci_remove_one_s4() (rmmod) paths because there are no calls to
lpfc_sli4_queue_unset() to kfree() the wq->pring.
Fix by inserting a call to lpfc_sli4_queue_unset() in
lpfc_sli_brdrestart_s4() and lpfc_sli4_hba_unset() routines. Also, add
a check for the SLI_ACTIVE flag before issuing the Q_DESTROY mailbox
command. If not set, then the mailbox command will obviously fail. In
such cases, skip issuing the mailbox command and only execute the driver
resource clean up portions of the lpfc_*q_destroy routines.

Fix a use-after-free bug in sg_release(), detected by syzbot with KASAN:
BUG: KASAN: slab-use-after-free in lock_release+0x151/0xa30
kernel/locking/lockdep.c:5838
__mutex_unlock_slowpath+0xe2/0x750 kernel/locking/mutex.c:912
sg_release+0x1f4/0x2e0 drivers/scsi/sg.c:407
In sg_release(), the function kref_put(&sfp->f_ref, sg_remove_sfp) is
called before releasing the open_rel_lock mutex. The kref_put() call may
decrement the reference count of sfp to zero, triggering its cleanup
through sg_remove_sfp(). This cleanup includes scheduling deferred work
via sg_remove_sfp_usercontext(), which ultimately frees sfp.
After kref_put(), sg_release() continues to unlock open_rel_lock and may
reference sfp or sdp. If sfp has already been freed, this results in a
slab-use-after-free error.
Move the kref_put(&sfp->f_ref, sg_remove_sfp) call after unlocking the
open_rel_lock mutex. This ensures:
- No references to sfp or sdp occur after the reference count is
decremented.
- Cleanup functions such as sg_remove_sfp() and
sg_remove_sfp_usercontext() can safely execute without impacting the
mutex handling in sg_release().
The fix has been tested and validated by syzbot. This patch closes the
bug reported at the following syzkaller link and ensures proper
sequencing of resource cleanup and mutex operations, eliminating the
risk of use-after-free errors in sg_release().

In 2010, runtime power management support was implemented in the SCSI
core. The description of patch "[SCSI] implement runtime Power
Management" mentions that the sg driver is skipped but not why. This
patch enables runtime power management even if an instance of the sg
driver is held open. Enabling runtime PM for the sg driver is safe
because all interactions of the sg driver with the SCSI device pass
through the block layer (blk_execute_rq_nowait()) and the block layer
already supports runtime PM.

We have two reports of failed memory allocation in btrfs' code which is
calling into report zones.
Both of these reports have the following signature coming from
__vmalloc_area_node():
kworker/u17:5: vmalloc error: size 0, failed to allocate pages, mode:0x10dc2(GFP_KERNEL|__GFP_HIGHMEM|__GFP_NORETRY|__GFP_ZERO), nodemask=(null),cpuset=/,mems_allowed=0
Further debugging showed these where allocations of one sector (512
bytes) and at least one of the reporter's systems where low on memory,
so going through the overhead of allocating a vm area failed.
Switching the allocation from __vmalloc() to kvzalloc() avoids the
overhead of vmalloc() on small allocations and succeeds.
Note: the buffer is already freed using kvfree() so there's no need to
adjust the free path.

During HBA stress testing, a spam of received PLOGIs exposes a resource
recovery bug causing leakage of lpfc_sqlq entries from the global
phba->sli4_hba.lpfc_els_sgl_list.
The issue is in lpfc_els_flush_cmd(), where the driver attempts to recover
outstanding ELS sgls when walking the txcmplq. Only CMD_ELS_REQUEST64_CRs
and CMD_GEN_REQUEST64_CRs are added to the abort and cancel lists. A check
for CMD_XMIT_ELS_RSP64_WQE is missing in order to recover LS_ACC usages of
the phba->sli4_hba.lpfc_els_sgl_list too.
Fix by adding CMD_XMIT_ELS_RSP64_WQE as part of the txcmplq walk when
adding WQEs to the abort and cancel list in lpfc_els_flush_cmd(). Also,
update naming convention from CRs to WQEs.

The SCSI disk message "Starting disk" to signal resuming of a suspended

in this message being printed twice when resuming from e.g. autosuspend:

[ 4972.805851] sd 0:0:0:0: [sda] Starting disk
[ 4980.558806] sd 0:0:0:0: [sda] Starting disk
Fix this double print by removing the call to sd_printk() from sd_resume()
and moving the call to sd_printk() in sd_resume_common() earlier in the
function, before the check using sd_do_start_stop(). Doing so, the message
is printed once regardless if sd_resume_common() actually executes
sd_start_stop_device() (i.e. SCSI device case) or not (libsas and libata
managed ATA devices case).

[ Upstream commit 6e5860b0ad ]
struct aac_srb_unit contains struct aac_srb, which contains struct sgmap,
which ends in a (currently) "fake" (1-element) flexible array. Converting
this to a flexible array is needed so that runtime bounds checking won't
think the array is fixed size (i.e. under CONFIG_FORTIFY_SOURCE=y and/or
CONFIG_UBSAN_BOUNDS=y), as other parts of aacraid use struct sgmap as a
flexible array.
It is not legal to have a flexible array in the middle of a structure, so
it either needs to be split up or rearranged so that it is at the end of
the structure. Luckily, struct aac_srb_unit, which is exclusively
consumed/updated by aac_send_safw_bmic_cmd(), does not depend on member
ordering.
The values set in the on-stack struct aac_srb_unit instance "srbu" by the
only two callers, aac_issue_safw_bmic_identify() and
aac_get_safw_ciss_luns(), do not contain anything in srbu.srb.sgmap.sg, and
they both implicitly initialize srbu.srb.sgmap.count to 0 during
memset(). For example:
memset(&srbu, 0, sizeof(struct aac_srb_unit));
srbcmd = &srbu.srb;
srbcmd->flags = cpu_to_le32(SRB_DataIn);
srbcmd->cdb[0] = CISS_REPORT_PHYSICAL_LUNS;
srbcmd->cdb[1] = 2; /* extended reporting */
srbcmd->cdb[8] = (u8)(datasize >> 8);
srbcmd->cdb[9] = (u8)(datasize);
rcode = aac_send_safw_bmic_cmd(dev, &srbu, phys_luns, datasize);
During aac_send_safw_bmic_cmd(), a separate srb is mapped into DMA, and has
srbu.srb copied into it:
srb = fib_data(fibptr);
memcpy(srb, &srbu->srb, sizeof(struct aac_srb));
Only then is srb.sgmap.count written and srb->sg populated:
srb->count = cpu_to_le32(xfer_len);
sg64 = (struct sgmap64 *)&srb->sg;
sg64->count = cpu_to_le32(1);
sg64->sg[0].addr[1] = cpu_to_le32(upper_32_bits(addr));
sg64->sg[0].addr[0] = cpu_to_le32(lower_32_bits(addr));
sg64->sg[0].count = cpu_to_le32(xfer_len);
But this is happening in the DMA memory, not in srbu.srb. An attempt to
copy the changes back to srbu does happen:
/*
* Copy the updated data for other dumping or other usage if
* needed
*/
memcpy(&srbu->srb, srb, sizeof(struct aac_srb));
But this was never correct: the sg64 (3 u32s) overlap of srb.sg (2 u32s)
always meant that srbu.srb would have held truncated information and any
attempt to walk srbu.srb.sg.sg based on the value of srbu.srb.sg.count
would result in attempting to parse past the end of srbu.srb.sg.sg[0] into
srbu.srb_reply.
After getting a reply from hardware, the reply is copied into
srbu.srb_reply:
srb_reply = (struct aac_srb_reply *)fib_data(fibptr);
memcpy(&srbu->srb_reply, srb_reply, sizeof(struct aac_srb_reply));
This has always been fixed-size, so there's no issue here. It is worth
noting that the two callers _never check_ srbu contents -- neither
srbu.srb nor srbu.srb_reply is examined. (They depend on the mapped
xfer_buf instead.)
Therefore, the ordering of members in struct aac_srb_unit does not matter,
and the flexible array member can moved to the end.
(Additionally, the two memcpy()s that update srbu could be entirely
removed as they are never consumed, but I left that as-is.)
