9646443f28 blk-mq: use quiesced elevator switch when reinitializing queues
[ Upstream commit 8237c01f16 ]

The hctx's run_work may be racing with the elevator switch when
reinitializing hardware queues. The queue is merely frozen in this
context, but that only prevents requests from allocating and doesn't
stop the hctx work from running. The work may get an elevator pointer
that's being torn down, and can result in use-after-free errors and
kernel panics (example below). Use the quiesced elevator switch instead,
and make the previous one static since it is now only used locally.

872e474f78 blk-mq-debugfs: add missing debugfs_mutex in blk_mq_debugfs_register_hctxs()
[ Upstream commit 9d20fd6ce1 ]

In blk_mq_update_nr_hw_queues(), debugfs_mutex is not held while
creating debugfs entries for hctxs. Hence add debugfs_mutex there,
it's safe because queue is not frozen.

0561aa6033 blk-cgroup: Reinit blkg_iostat_set after clearing in blkcg_reset_stats()
[ Upstream commit 3d2af77e31 ]

When blkg_alloc() is called to allocate a blkcg_gq structure
with the associated blkg_iostat_set's, there are 2 fields within
blkg_iostat_set that requires proper initialization - blkg & sync.
The former field was introduced by commit 3b8cc62987 ("blk-cgroup:
Optimize blkcg_rstat_flush()") while the later one was introduced by
commit f733164829 ("blk-cgroup: reimplement basic IO stats using
cgroup rstat").

Unfortunately those fields in the blkg_iostat_set's are not properly
re-initialized when they are cleared in v1's blkcg_reset_stats(). This
can lead to a kernel panic due to NULL pointer access of the blkg
pointer. The missing initialization of sync is less problematic and
can be a problem in a debug kernel due to missing lockdep initialization.

Fix these problems by re-initializing them after memory clearing.

2f945c9892 block: rate-limit capacity change info log
commit 3179a5f7f8 upstream.

loop devices under heavy stress-ng loop streessor can trigger many
capacity change events in a short time. Each event prints an info
message from set_capacity_and_notify(), flooding the console and
contributing to soft lockups on slow consoles.

Switch the printk in set_capacity_and_notify() to
pr_info_ratelimited() so frequent capacity changes do not spam
the log while still reporting occasional changes.

0e2d3a8d9f Revert "block: don't add or resize partition on the disk with GENHD_FL_NO_PART"
This reverts commit 1a721de848.

The commit 1a721de848 ("block: don't add or resize partition on the disk
with GENHD_FL_NO_PART") and the commit 7777f47f2e ("block: Move checking
GENHD_FL_NO_PART to bdev_add_partition()") used the flag GENHD_FL_NO_PART
to prevent the add or resize of partitions in 5.15 stable kernels.But in
these 5.15 kernels, this is giving an issue with the following error
where the loop driver wants to create a partition when the partscan is
disabled on the loop device:

dd if=/dev/zero of=loopDisk.dsk bs=1M count=1 seek=10240;
losetup -f loopDisk.dsk;parted -s /dev/loop0 -- mklabel gpt mkpart primary

Error: Partition(s) 1 on /dev/loop0 have been written, but we have been
unable to inform the kernel of the change, probably because it/they are
in use. As a result, the old partition(s) will remain in use. You should
reboot now before making further changes.
""
If the partition scan is not enabled on the loop device, this flag
GENHD_FL_NO_PART is getting set and when partition creation is tried,
it returns an error EINVAL thereby preventing the creation of partitions.
So, there is no such distinction between disabling of partition scan and
partition creation.

Later in 6.xxx kernels, the commit b9684a71fc ("block, loop: support
partitions without scanning") a new flag GD_SUPPRESS_PART_SCAN was
introduced that just disables the partition scan and uses GENHD_FL_NO_PART
only to prevent creating partition scan. So, the partition creationg can
proceed with even if partition scan is disabled.

As the commit b9684a71fc ("block, loop: support partitions without
scanning") is not available in 5.15 stable kernel, and since there is no
distinction between disabling of "partition scan" and "partition
creation", we need to revert the commits 1a721de848 and 7777f47f2e
from 5.15 stable kernel to allow partition creation when partscan is
disabled.

22863772e9 block: use int to store blk_stack_limits() return value
[ Upstream commit b0b4518c99 ]

Change the 'ret' variable in blk_stack_limits() from unsigned int to int,
as it needs to store negative value -1.

Storing the negative error codes in unsigned type, or performing equality
comparisons (e.g., ret == -1), doesn't cause an issue at runtime [1] but
can be confusing. Additionally, assigning negative error codes to unsigned
type may trigger a GCC warning when the -Wsign-conversion flag is enabled.

No effect on runtime.

The warn in this situation is meaningless. Since this iocg is being
removed, the state of the 'active_list' is irrelevant, and 'waitq_timer'
is canceled after removing 'active_list' in ioc_pd_free(), which ensures
iocg is freed after iocg_waitq_timer_fn() returns.

Therefore, add the check if iocg was already offlined to avoid warn
when removing a blkcg or disk.

d20f28f007 blk-cgroup: support to track if policy is online
commit dfd6200a09 upstream.

A new field 'online' is added to blkg_policy_data to fix following
2 problem:

1) In blkcg_activate_policy(), if pd_alloc_fn() with 'GFP_NOWAIT'
   failed, 'queue_lock' will be dropped and pd_alloc_fn() will try again
   without 'GFP_NOWAIT'. In the meantime, remove cgroup can race with
   it, and pd_offline_fn() will be called without pd_init_fn() and
   pd_online_fn(). This way null-ptr-deference can be triggered.

2) In order to synchronize pd_free_fn() from blkg_free_workfn() and
   blkcg_deactivate_policy(), 'list_del_init(&blkg->q_node)' will be
   delayed to blkg_free_workfn(), hence pd_offline_fn() can be called
   first in blkg_destroy(), and then blkcg_deactivate_policy() will
   call it again, we must prevent it.

The new field 'online' will be set after pd_online_fn() and will be
cleared after pd_offline_fn(), in the meantime pd_offline_fn() will only
be called if 'online' is set.

51e0101ab9 block: fix conversion of GPT partition name to 7-bit
commit e06472bab2 upstream.

The utf16_le_to_7bit function claims to, naively, convert a UTF-16
string to a 7-bit ASCII string. By naively, we mean that it:
 * drops the first byte of every character in the original UTF-16 string
 * checks if all characters are printable, and otherwise replaces them
   by exclamation mark "!".

This means that theoretically, all characters outside the 7-bit ASCII
range should be replaced by another character. Examples:

 * lower-case alpha (ɒ) 0x0252 becomes 0x52 (R)
 * ligature OE (œ) 0x0153 becomes 0x53 (S)
 * hangul letter pieup (ㅂ) 0x3142 becomes 0x42 (B)
 * upper-case gamma (Ɣ) 0x0194 becomes 0x94 (not printable) so gets
   replaced by "!"

The result of this conversion for the GPT partition name is passed to
user-space as PARTNAME via udev, which is confusing and feels questionable.

However, there is a flaw in the conversion function itself. By dropping
one byte of each character and using isprint() to check if the remaining
byte corresponds to a printable character, we do not actually guarantee
that the resulting character is 7-bit ASCII.

This happens because we pass 8-bit characters to isprint(), which
in the kernel returns 1 for many values > 0x7f - as defined in ctype.c.

This results in many values which should be replaced by "!" to be kept
as-is, despite not being valid 7-bit ASCII. Examples:

 * e with acute accent (é) 0x00E9 becomes 0xE9 - kept as-is because
   isprint(0xE9) returns 1.
 * euro sign (€) 0x20AC becomes 0xAC - kept as-is because isprint(0xAC)
   returns 1.

This way has broken pyudev utility[1], fixes it by using a mask of 7 bits
instead of 8 bits before calling isprint.

40a35d14f3 partitions: mac: fix handling of bogus partition table
commit 80e648042e upstream.

Fix several issues in partition probing:

 - The bailout for a bad partoffset must use put_dev_sector(), since the
   preceding read_part_sector() succeeded.
 - If the partition table claims a silly sector size like 0xfff bytes
   (which results in partition table entries straddling sector boundaries),
   bail out instead of accessing out-of-bounds memory.
 - We must not assume that the partition table contains proper NUL
   termination - use strnlen() and strncmp() instead of strlen() and
   strcmp().

2d43a6fcea block: retry call probe after request_module in blk_request_module

Set kernel config:

 CONFIG_BLK_DEV_LOOP=m
 CONFIG_BLK_DEV_LOOP_MIN_COUNT=0

Do latter:

 mknod loop0 b 7 0
 exec 4<> loop0

Before commit e418de3abc ("block: switch gendisk lookup to a simple
xarray"), lookup_gendisk will first use base_probe to load module loop,
and then the retry will call loop_probe to prepare the loop disk. Finally
open for this disk will success. However, after this commit, we lose the
retry logic, and open will fail with ENXIO. Block device autoloading is
deprecated and will be removed soon, but maybe we should keep open success
until we really remove it. So, give a retry to fix it.

commit 1ba0403ac6 ("block, bfq: fix uaf for accessing waker_bfqq after
splitting") fix the problem that if waker_bfqq is in the merge chain,
and current is the only procress, waker_bfqq can be freed from
bfq_split_bfqq(). However, the case that waker_bfqq is not in the merge
chain is missed, and if the procress reference of waker_bfqq is 0,
waker_bfqq can be freed as well.

Fix the problem by checking procress reference if waker_bfqq is not in
the merge_chain.

__propagate_weights() is called with an active value of zero in
ioc_check_iocgs(), which results in the high value being less than the
low value, which is undefined because the value returned depends on the
order of the comparisons.

The purpose of this expression is to ensure inuse is not more than
active and at least 1. This could be written more simply with a ternary
expression that uses min(inuse, active) as the condition so that the
value of that condition can be used if it is not zero and one if it is.
Do this conversion to resolve the error and add a comment to deter
people from turning this back into clamp().

Both scenarios are similar, the full memory barrier should be inserted
between 1) and 2), as well as between 3) and 4) to make sure that either
CPU0 sees BLK_MQ_S_STOPPED is cleared or CPU1 sees dispatch list.
Otherwise, either CPU will not rerun the hardware queue causing
starvation of the request.

The easy way to fix it is to add the essential full memory barrier into
helper of blk_mq_hctx_stopped(). In order to not affect the fast path
(hardware queue is not stopped most of the time), we only insert the
barrier into the slow path. Actually, only slow path needs to care about
missing of dispatching the request to the low-level device driver.

f8b7e56b75 block: Fix elevator_get_default() checking for NULL q->tag_set
[ Upstream commit b402328a24 ]

elevator_get_default() and elv_support_iosched() both check for whether

Remove the checks, which both simplifies the logic and avoids checker

So rq_qos_wake_function() calls wake_up_process(data->task), which calls
try_to_wake_up(), which faults in raw_spin_lock_irqsave(&p->pi_lock).

p comes from data->task, and data comes from the waitqueue entry, which
is stored on the waiter's stack in rq_qos_wait(). Analyzing the core
dump with drgn, I found that the waiter had already woken up and moved
on to a completely unrelated code path, clobbering what was previously
data->task. Meanwhile, the waker was passing the clobbered garbage in
data->task to wake_up_process(), leading to the crash.

What's happening is that in between rq_qos_wake_function() deleting the
waitqueue entry and calling wake_up_process(), rq_qos_wait() is finding
that it already got a token and returning. The race looks like this:

rq_qos_wait()                           rq_qos_wake_function()
==============================================================
prepare_to_wait_exclusive()
                                        data->got_token = true;
                                        list_del_init(&curr->entry);
if (data.got_token)
        break;
finish_wait(&rqw->wait, &data.wq);
  ^- returns immediately because
     list_empty_careful(&wq_entry->entry)
     is true
... return, go do something else ...
                                        wake_up_process(data->task)
                                          (NO LONGER VALID!)-^

Normally, finish_wait() is supposed to synchronize against the waker.
But, as noted above, it is returning immediately because the waitqueue
entry has already been removed from the waitqueue.

The bug is that rq_qos_wake_function() is accessing the waitqueue entry
AFTER deleting it. Note that autoremove_wake_function() wakes the waiter
and THEN deletes the waitqueue entry, which is the proper order.

Fix it by swapping the order. We also need to use
list_del_init_careful() to match the list_empty_careful() in
finish_wait().

63a07379fd block, bfq: fix uaf for accessing waker_bfqq after splitting
commit 1ba0403ac6 upstream.

After commit 42c306ed72 ("block, bfq: don't break merge chain in

the bfqq can be freed after bfq_split_bfqq(). Hence recored the bfqq and
then access bfqq->waker_bfqq may trigger UAF. What's more, the waker_bfqq
may in the merge chain of bfqq, hence just recored waker_bfqq is still
not safe.

bfqq->waker_bfqq is in the merge chain, and current procress is the only

cc4d21d949 block: fix potential invalid pointer dereference in blk_add_partition
[ Upstream commit 26e197b7f9 ]

The blk_add_partition() function initially used a single if-condition
(IS_ERR(part)) to check for errors when adding a partition. This was
modified to handle the specific case of -ENXIO separately, allowing the
function to proceed without logging the error in this case. However,
this change unintentionally left a path where md_autodetect_dev()
could be called without confirming that part is a valid pointer.

This commit separates the error handling logic by splitting the
initial if-condition, improving code readability and handling specific
error scenarios explicitly. The function now distinguishes the general
error case from -ENXIO without altering the existing behavior of
md_autodetect_dev() calls.

e0c20d88b7 block, bfq: don't break merge chain in bfq_split_bfqq()
[ Upstream commit 42c306ed72 ]

Consider the following scenario:

    Process 1       Process 2       Process 3       Process 4
     (BIC1)          (BIC2)          (BIC3)          (BIC4)
      Λ               |               |                |
       \-------------\ \-------------\ \--------------\|

ref    0              1               2                4

If Process 1 issue a new IO and bfqq2 is found, and then bfq_init_rq()
decide to spilt bfqq2 by bfq_split_bfqq(). Howerver, procress reference
of bfqq2 is 1 and bfq_split_bfqq() just clear the coop flag, which will
break the merge chain.

Expected result: caller will allocate a new bfqq for BIC1

    Process 1       Process 2       Process 3       Process 4
     (BIC1)          (BIC2)          (BIC3)          (BIC4)
                      |               |                |
                       \-------------\ \--------------\|
                                      V                V
      bfqq1--------->bfqq2---------->bfqq3----------->bfqq4
ref    0              0               1                3

Since the condition is only used for the last bfqq4 when the previous
bfqq2 and bfqq3 are already splited. Fix the problem by checking if
bfqq is the last one in the merge chain as well.

3883a6873b block, bfq: choose the last bfqq from merge chain in bfq_setup_cooperator()
[ Upstream commit 0e456dba86 ]

Consider the following merge chain:

Process 1       Process 2       Process 3	Process 4
 (BIC1)          (BIC2)          (BIC3)		 (BIC4)
  Λ                |               |               |
   \--------------\ \-------------\ \-------------\|
                   V               V		   V
  bfqq1--------->bfqq2---------->bfqq3----------->bfqq4

IO from Process 1 will get bfqf2 from BIC1 first, then
bfq_setup_cooperator() will found bfqq2 already merged to bfqq3 and then
handle this IO from bfqq3. However, the merge chain can be much deeper
and bfqq3 can be merged to other bfqq as well.

Fix this problem by iterating to the last bfqq in
bfq_setup_cooperator().

8aa9de02a4 block, bfq: fix possible UAF for bfqq->bic with merge chain
[ Upstream commit 18ad4df091 ]

1) initial state, three tasks:

		Process 1       Process 2	Process 3
		 (BIC1)          (BIC2)		 (BIC3)
		  |  Λ            |  Λ		  |  Λ

		  bfqq1           bfqq2		  bfqq3
process ref:	   1		    1		    1

2) bfqq1 merged to bfqq2:

		Process 1       Process 2	Process 3
		 (BIC1)          (BIC2)		 (BIC3)
		  |               |		  |  Λ
		  \--------------\|		  |  |
		                  V		  V  |
		  bfqq1--------->bfqq2		  bfqq3
process ref:	   0		    2		    1

3) bfqq2 merged to bfqq3:

		Process 1       Process 2	Process 3
		 (BIC1)          (BIC2)		 (BIC3)
	 here -> Λ                |		  |

		                  V		  V
		  bfqq1--------->bfqq2---------->bfqq3
process ref:	   0		    1		    3

In this case, IO from Process 1 will get bfqq2 from BIC1 first, and then
get bfqq3 through merge chain, and finially handle IO by bfqq3.
Howerver, current code will think bfqq2 is owned by BIC1, like initial
state, and set bfqq2->bic to BIC1.

bfq_insert_request
-> by Process 1
 bfqq = bfq_init_rq(rq)
  bfqq = bfq_get_bfqq_handle_split
   bfqq = bic_to_bfqq
   -> get bfqq2 from BIC1
 bfqq->ref++
 rq->elv.priv[0] = bic
 rq->elv.priv[1] = bfqq
 if (bfqq_process_refs(bfqq) == 1)
  bfqq->bic = bic
  -> record BIC1 to bfqq2

  __bfq_insert_request
   new_bfqq = bfq_setup_cooperator
   -> get bfqq3 from bfqq2->new_bfqq
   bfqq_request_freed(bfqq)
   new_bfqq->ref++
   rq->elv.priv[1] = new_bfqq
   -> handle IO by bfqq3

Fix the problem by checking bfqq is from merge chain fist. And this
might fix a following problem reported by our syzkaller(unreproducible):

Now that there are no indirect calls for PI processing there is no
way to dereference a NULL pointer here. Additionally drivers now always

This is effectively a revert of commit 3df49967f6 ("block: flush the

cf6b45ea7a block: initialize integrity buffer to zero before writing it to media
[ Upstream commit 899ee2c382 ]

Metadata added by bio_integrity_prep is using plain kmalloc, which leads
to random kernel memory being written media. For PI metadata this is
limited to the app tag that isn't used by kernel generated metadata,
but for non-PI metadata the entire buffer leaks kernel memory.

Fix this by adding the __GFP_ZERO flag to allocations for writes.

Historically, the signed integer overflow sanitizer did not work in the
kernel due to its interaction with `-fwrapv` but this has since been
changed [1] in the newest version of Clang; It was re-enabled in the
kernel with Commit 557f8c582a ("ubsan: Reintroduce signed overflow
sanitizer").

Let's rework this overflow checking logic to not actually perform an
overflow during the check itself, thus avoiding the UBSAN splat.

844fc023e9 blk-iocost: avoid out of bounds shift
[ Upstream commit beaa51b360 ]

UBSAN catches undefined behavior in blk-iocost, where sometimes
iocg->delay is shifted right by a number that is too large,
resulting in undefined behavior on some architectures.

Avoid that undefined behavior by simply taking the
"delay = 0" branch if the shift is too large.

I am not sure what the symptoms of an undefined value
delay will be, but I suspect it could be more than a
little annoying to debug.

d05e6ba1b7 block: Clear zone limits for a non-zoned stacked queue
[ Upstream commit c8f6f88d25 ]

Device mapper may create a non-zoned mapped device out of a zoned device
(e.g., the dm-zoned target). In such case, some queue limit such as the
max_zone_append_sectors and zone_write_granularity endup being non zero
values for a block device that is not zoned. Avoid this by clearing
these limits in blk_stack_limits() when the stacked zoned limit is
false.

f6cbb4843c block: sed-opal: handle empty atoms when parsing response
[ Upstream commit 5429c8de56 ]

The SED Opal response parsing function response_parse() does not
handle the case of an empty atom in the response. This causes
the entry count to be too high and the response fails to be
parsed. Recognizing, but ignoring, empty atoms allows response
handling to succeed.

1e4d3f8bd8 blk-iocost: Fix an UBSAN shift-out-of-bounds warning
[ Upstream commit 2a427b49d0 ]

When iocg_kick_delay() is called from a CPU different than the one which set
the delay, @now may be in the past of @iocg->delay_at leading to the
following warning:

  UBSAN: shift-out-of-bounds in block/blk-iocost.c:1359:23

The underflow itself doesn't really affect the behavior in any meaningful
way; however, the past timestamp may exaggerate the delay amount calculated
later in the code, which shouldn't be a material problem given the nature of
the delay mechanism.

If @now is in the past, this CPU is racing another CPU which recently set up
the delay and there's nothing this CPU can contribute w.r.t. the delay.
Let's bail early from iocg_kick_delay() in such cases.
