commit 8259cb14a7 upstream.
RV per-task monitors are implemented through a monitor structure
available for each task_struct. This structure is reset every time the
monitor is (re-)started, to avoid inconsistencies if the monitor was
activated previously.
To do so, we reset the monitor on all threads using the macro
for_each_process_thread. However, this macro excludes the idle tasks on
each CPU. Idle tasks could be considered tasks on their own right and it
should be up to the model whether to ignore them or not.
Reset monitors also on the idle tasks for each present CPU whenever we
reset all per-task monitors.

commit 1e7381f361 upstream.
Explicitly verify the target vCPU is fully online _prior_ to clamping the
index in kvm_get_vcpu(). If the index is "bad", the nospec clamping will
generate '0', i.e. KVM will return vCPU0 instead of NULL.
In practice, the bug is unlikely to cause problems, as it will only come
into play if userspace or the guest is buggy or misbehaving, e.g. KVM may
send interrupts to vCPU0 instead of dropping them on the floor.
However, returning vCPU0 when it shouldn't exist per online_vcpus is
problematic now that KVM uses an xarray for the vCPUs array, as KVM needs
to insert into the xarray before publishing the vCPU to userspace (see
commit c5b0775491 ("KVM: Convert the kvm->vcpus array to a xarray")),
i.e. before vCPU creation is guaranteed to succeed.
As a result, incorrectly providing access to vCPU0 will trigger a
use-after-free if vCPU0 is dereferenced and kvm_vm_ioctl_create_vcpu()
bails out of vCPU creation due to an error and frees vCPU0. Commit
afb2acb2e3 ("KVM: Fix vcpu_array[0] races") papered over that issue, but
in doing so introduced an unsolvable teardown conundrum. Preventing
accesses to vCPU0 before it's fully online will allow reverting commit
afb2acb2e3, without re-introducing the vcpu_array[0] UAF race.

In case of tc offload, when user space queries the kernel for tc action
statistics, tc will query the offloaded statistics from device drivers.
Among other statistics, drivers are expected to pass the number of
packets that hit the action since the last query as a 64-bit number.
Unfortunately, tc treats the number of packets as a 32-bit number,
leading to truncation and incorrect statistics when the number of
packets since the last query exceeds 0xffffffff:
$ tc -s filter show dev swp2 ingress
filter protocol all pref 1 flower chain 0
filter protocol all pref 1 flower chain 0 handle 0x1
skip_sw
in_hw in_hw_count 1
action order 1: mirred (Egress Redirect to device swp1) stolen
index 1 ref 1 bind 1 installed 58 sec used 0 sec
Action statistics:
Sent 1133877034176 bytes 536959475 pkt (dropped 0, overlimits 0 requeues 0)
[...]
According to the above, 2111-byte packets were redirected which is
impossible as only 64-byte packets were transmitted and the MTU was
1500.
Fix by treating packets as a 64-bit number:
$ tc -s filter show dev swp2 ingress
filter protocol all pref 1 flower chain 0
filter protocol all pref 1 flower chain 0 handle 0x1
skip_sw
in_hw in_hw_count 1
action order 1: mirred (Egress Redirect to device swp1) stolen
index 1 ref 1 bind 1 installed 61 sec used 0 sec
Action statistics:
Sent 1370624380864 bytes 21416005951 pkt (dropped 0, overlimits 0 requeues 0)
[...]
Which shows that only 64-byte packets were redirected (1370624380864 /
21416005951 = 64).

And, consequently, two further fixes for things masked by the above bug:
(2) Fix rxrpc_input_conn_event() to handle being invoked with a NULL

Zbigniew mentioned at Linux Plumber's that systemd is interested in
switching to execveat() for service execution, but can't, because the
contents of /proc/pid/comm are the file descriptor which was used,
instead of the path to the binary[1]. This makes the output of tools like
top and ps useless, especially in a world where most fds are opened
CLOEXEC so the number is truly meaningless.
When the filename passed in is empty (e.g. with AT_EMPTY_PATH), use the
dentry's filename for "comm" instead of using the useless numeral from
the synthetic fdpath construction. This way the actual exec machinery
is unchanged, but cosmetically the comm looks reasonable to admins
investigating things.
Instead of adding TASK_COMM_LEN more bytes to bprm, use one of the unused
flag bits to indicate that we need to set "comm" from the dentry.

Kernel panic - not syncing: Oops - BUG: Fatal exception
This happens because pps_device_destruct() frees the pps_device with the
embedded cdev immediately after calling cdev_del(), but, as the comment
above cdev_del() notes, fops for previously opened cdevs are still
callable even after cdev_del() returns. I think this bug has always
been there: I can't explain why it suddenly started happening every time
I reboot this particular board.
In commit d953e0e837 ("pps: Fix a use-after free bug when
unregistering a source."), George Spelvin suggested removing the
embedded cdev. That seems like the simplest way to fix this, so I've
implemented his suggestion, using __register_chrdev() with pps_idr
becoming the source of truth for which minor corresponds to which
device.
But now that pps_idr defines userspace visibility instead of cdev_add(),
we need to be sure the pps->dev refcount can't reach zero while
userspace can still find it again. So, the idr_remove() call moves to
pps_unregister_cdev(), and pps_idr now holds a reference to pps->dev.

The Source can drop its output voltage to the minimum of the requested
PPS APDO voltage range when it is in Current Limit Mode. If this voltage
falls within the range of vPpsShutdown, the Source initiates a Hard
Reset and discharges Vbus. However, currently the Sink may disconnect
before the voltage reaches vPpsShutdown, leading to unexpected behavior.
Prevent premature disconnection by setting the Sink's disconnect
threshold to the minimum vPpsShutdown value. Additionally, consider the
voltage drop due to IR drop when calculating the appropriate threshold.
This ensures a robust and reliable interaction between the Source and
Sink during SPR PPS Current Limit Mode operation.

Patch series "Finish the create_empty_buffers() transition", v2.
Pankaj recently added folio_create_empty_buffers() as the folio equivalent
to create_empty_buffers(). This patch set finishes the conversion by
first converting all remaining filesystems to call
folio_create_empty_buffers(), then renaming it back to
create_empty_buffers(). I took the opportunity to make a few
simplifications like making folio_create_empty_buffers() return the head
buffer and extracting get_nth_bh() from nilfs2.
A few of the patches in this series aren't directly related to
create_empty_buffers(), but I saw them while I was working on this and
thought they'd be easy enough to add to this series. Compile-tested only,
other than ext4.
This patch (of 26):
Almost all callers want to know the first BH that was allocated for this
folio. We already have that handy, so return it.

The of_syscon_register_regmap() API allows an externally created regmap
to be registered with syscon. This regmap can then be returned to client
drivers using the syscon_regmap_lookup_by_phandle() APIs.
The API is used by platforms where mmio access to the syscon registers is
not possible, and a underlying soc driver like exynos-pmu provides a SoC
specific regmap that can issue a SMC or hypervisor call to write the
register.
This approach keeps the SoC complexities out of syscon, but allows common
drivers such as syscon-poweroff, syscon-reboot and friends that are used
by many SoCs already to be re-used.

The of_property_for_each_u32() macro needs five parameters, two of which
are primarily meant as internal variables for the macro itself (in the
for() clause). Yet these two parameters are used by a few drivers, and this
can be considered misuse or at least bad practice.
Now that the kernel uses C11 to build, these two parameters can be avoided
by declaring them internally, thus changing this pattern:
struct property *prop;
const __be32 *p;
u32 val;
of_property_for_each_u32(np, "xyz", prop, p, val) { ... }
to this:
u32 val;
of_property_for_each_u32(np, "xyz", val) { ... }
However two variables cannot be declared in the for clause even with C11,
so declare one struct that contain the two variables we actually need. As
the variables inside this struct are not meant to be used by users of this
macro, give the struct instance the noticeable name "_it" so it is visible
during code reviews, helping to avoid new code to use it directly.
Most usages are trivially converted as they do not use those two
parameters, as expected. The non-trivial cases are:
- drivers/clk/clk.c, of_clk_get_parent_name(): easily doable anyway
- drivers/clk/clk-si5351.c, si5351_dt_parse(): this is more complex as the
checks had to be replicated in a different way, making code more verbose
and somewhat uglier, but I refrained from a full rework to keep as much
of the original code untouched having no hardware to test my changes
All the changes have been build tested. The few for which I have the
hardware have been runtime-tested too.

Introduce a new PLL reset mode flag which controls whether or not to
reset a PLL after adjusting its rate. The mode can be configured through
platform data or device tree.
Since commit 6dc669a22c ("clk: si5351: Add PLL soft reset"), the
driver unconditionally resets a PLL whenever its rate is adjusted.
The rationale was that a PLL reset was required to get three outputs
working at the same time. Before this change, the driver never reset the
PLLs.
Commit b26ff127c5 ("clk: si5351: Apply PLL soft reset before enabling
the outputs") subsequently introduced an option to reset the PLL when
enabling a clock output that sourced it. Here, the rationale was that
this is required to get a deterministic phase relationship between
multiple output clocks.
This clearly shows that it is useful to reset the PLLs in applications
where multiple clock outputs are used. However, the Si5351 also allows
for glitch-free rate adjustment of its PLLs if one avoids resetting the
PLL. In our audio application where a single Si5351 clock output is used
to supply a runtime adjustable bit clock, this unconditional PLL reset
behaviour introduces unwanted glitches in the clock output.
It would appear that the problem being solved in the former commit
may be solved by using the optional device tree property introduced in
the latter commit, obviating the need for an unconditional PLL reset
after rate adjustment. But it's not OK to break the default behaviour of
the driver, and it cannot be assumed that all device trees are using the
property introduced in the latter commit. Hence, the new behaviour is
made opt-in.

If we manage to begin an async call, but fail to transmit any data on it
due to a signal, we then abort it which causes a race between the
notification of call completion from rxrpc and our attempt to cancel the
notification. The notification will be necessary, however, for async
FetchData to terminate the netfs subrequest.
However, since we get a notification from rxrpc upon completion of a call
(aborted or otherwise), we can just leave it to that.
This leads to calls not getting cleaned up, but appearing in
/proc/net/rxrpc/calls as being aborted with code 6.
Fix this by making the "error_do_abort:" case of afs_make_call() abort the
call and then abandon it to the notification handler.

commit 2f8dea1692 upstream.
Consider a scenario where a CPU transitions from CPUHP_ONLINE to halfway
through a CPU hotunplug down to CPUHP_HRTIMERS_PREPARE, and then back to
CPUHP_ONLINE:
Since hrtimers_prepare_cpu() does not run, cpu_base.hres_active remains set
to 1 throughout. However, during a CPU unplug operation, the tick and the
clockevents are shut down at CPUHP_AP_TICK_DYING. On return to the online
state, for instance CFS incorrectly assumes that the hrtick is already
active, and the chance of the clockevent device to transition to oneshot
mode is also lost forever for the CPU, unless it goes back to a lower state
than CPUHP_HRTIMERS_PREPARE once.
This round-trip reveals another issue; cpu_base.online is not set to 1
after the transition, which appears as a WARN_ON_ONCE in enqueue_hrtimer().
Aside of that, the bulk of the per CPU state is not reset either, which
means there are dangling pointers in the worst case.
Address this by adding a corresponding startup() callback, which resets the
stale per CPU state and sets the online flag.
[ tglx: Make the new callback unconditionally available, remove the online
modification in the prepare() callback and clear the remaining

As the comment above waitqueue_active() explains, it can only be used
if both waker and waiter have mb()'s that pair with each other. However
__pollwait() is broken in this respect.
This is not pipe-specific, but let's look at pipe_poll() for example:
poll_wait(...); // -> __pollwait() -> add_wait_queue()
LOAD(pipe->head);
LOAD(pipe->head);
In theory these LOAD()'s can leak into the critical section inside
add_wait_queue() and can happen before list_add(entry, wq_head), in this
case pipe_poll() can race with wakeup_pipe_readers/writers which do
smp_mb();
if (waitqueue_active(wq_head))
wake_up_interruptible(wq_head);
There are more __pollwait()-like functions (grep init_poll_funcptr), and
it seems that at least ep_ptable_queue_proc() has the same problem, so the
patch adds smp_mb() into poll_wait().

The issue may be triggered by damon, offline_page, page_idle, etc, which
will increase the refcount of page table.
1. The page table itself will be discarded after reporting the
"nonzero mapcount".
2. The HugeTLB page mapped by the page table miss freeing since we
treat the page table as shared and a shared page table will not be
unmapped.
Fix it by introducing independent PMD page table shared count. As
described by comment, pt_index/pt_mm/pt_frag_refcount are used for s390
gmap, x86 pgds and powerpc, pt_share_count is used for x86/arm64/riscv
pmds, so we can reuse the field as pt_share_count.

Sharing page tables between processes but falling back to per-MM page
table locks cannot possibly work.
So, let's make sure that we do have split PMD locks by adding a new
Kconfig option and letting that depend on CONFIG_SPLIT_PMD_PTLOCKS.

commit 7bac656875 upstream.
PHY might already be powered on during ufs_qcom_power_up_sequence() in a
couple of cases:
1. During UFSHCD_QUIRK_REINIT_AFTER_MAX_GEAR_SWITCH quirk
2. Resuming from spm_lvl = 5 suspend
In those cases, it is necessary to call phy_power_off() and phy_exit() in
ufs_qcom_power_up_sequence() function to power off the PHY before calling
phy_init() and phy_power_on().
Case (1) is doing it via ufs_qcom_reinit_notify() callback, but case (2) is
not handled. So to satisfy both cases, call phy_power_off() and phy_exit()
if the phy_count is non-zero. And with this change, the reinit_notify()
callback is no longer needed.
This fixes the below UFS resume failure with spm_lvl = 5:

[ Upstream commit 3479c7549f ]
If the backlog of listen() is set to zero, sk_acceptq_is_full() allows
one connection to be made, but inet_csk_reqsk_queue_is_full() does not.
When the net.ipv4.tcp_syncookies is zero, inet_csk_reqsk_queue_is_full()
will cause an immediate drop before the sk_acceptq_is_full() check in
tcp_conn_request(), resulting in no connection can be made.
This patch tries to keep consistent with 64a146513f ("[NET]: Revert
incorrect accept queue backlog changes.").

locklessly, there is one more function mostly called in TCP/DCCP

commit 8043832e2a upstream.
Introduce numa_valid_node(nid) that verifies that nid is a valid node ID
and use that instead of comparing nid parameter with either NUMA_NO_NODE
or MAX_NUMNODES.
This makes the checks for valid node IDs consistent and more robust and
allows to get rid of multiple WARNings.

commit bcc80dec91 upstream.
read_hv_sched_clock_tsc() assumes that the Hyper-V clock counter is
bigger than the variable hv_sched_clock_offset, which is cached during
early boot, but depending on the timing this assumption may be false
when a hibernated VM starts again (the clock counter starts from 0
again) and is resuming back (Note: hv_init_tsc_clocksource() is not
called during hibernation/resume); consequently,
read_hv_sched_clock_tsc() may return a negative integer (which is
interpreted as a huge positive integer since the return type is u64)
and new kernel messages are prefixed with huge timestamps before
read_hv_sched_clock_tsc() grows big enough (which typically takes
several seconds).
Fix the issue by saving the Hyper-V clock counter just before the
suspend, and using it to correct the hv_sched_clock_offset in
resume. This makes hv tsc page based sched_clock continuous and ensures
that post resume, it starts from where it left off during suspend.
Override x86_platform.save_sched_clock_state and
x86_platform.restore_sched_clock_state routines to correct this as soon
as possible.
Note: if Invariant TSC is available, the issue doesn't happen because
1) we don't register read_hv_sched_clock_tsc() for sched clock:
See commit e5313f1c54 ("clocksource/drivers/hyper-v: Rework
clocksource and sched clock setup");
2) the common x86 code adjusts TSC similarly: see
__restore_processor_state() -> tsc_verify_tsc_adjust(true) and
x86_platform.restore_sched_clock_state().
