0bc22ecade x86/PCI: Use host bridge _CRS info on Foxconn K8M890-8237A
commit 1dace0116d upstream.

The Foxconn K8M890-8237A has two PCI host bridges, and we can't assign
resources correctly without the information from _CRS that tells us which
address ranges are claimed by which bridge. In the bugs mentioned below,
we incorrectly assign a sound card address (this example is from 1033299):

  bus: 00 index 2 [mem 0x80000000-0xfcffffffff]
ACPI: PCI Root Bridge [PCI0] (domain 0000 [bus 00-7f])

We assigned 0xfd_0000_0000, but that is not in any of the host bridge
windows, and the sound card doesn't work.

Turn on pci=use_crs automatically for this system.

19dd3b1d21 powerpc/perf: Fix book3s kernel to userspace backtraces
commit 72e349f112 upstream.

When we take a PMU exception or a software event we call
perf_read_regs(). This overloads regs->result with a boolean that
describes if we should use the sampled instruction address register
(SIAR) or the regs.

If the exception is in kernel, we start with the kernel regs and
backtrace through the kernel stack. At this point we switch to the
userspace regs and backtrace the user stack with perf_callchain_user().

Unfortunately these regs have not got the perf_read_regs() treatment,
so regs->result could be anything. If it is non zero,
perf_instruction_pointer() decides to use the SIAR, and we get issues
like this:

0.11% qemu-system-ppc [kernel.kallsyms] [k] _raw_spin_lock_irqsave

          | | | 0x7e714

Notice the bogus _raw_spin_irqsave when we transition from kernel
(system_call) to userspace (0x7e714). We inserted what was in the SIAR.

Add a check in regs_use_siar() to check that the regs in question
are from a PMU exception. With this fix the backtrace makes sense:

ce31b27e12 x86/mce: Fix MCE severity messages
commit 17fea54bf0 upstream.

Derek noticed that a critical MCE gets reported with the wrong
error type description:

[Hardware Error]: CPU 34: Machine Check Exception: 5 Bank 9: f200003f000100b0
  [Hardware Error]: RIP !INEXACT! 10:<ffffffff812e14c1> {intel_idle+0xb1/0x170}

[Hardware Error]: Some CPUs didn't answer in synchronization
  [Hardware Error]: Machine check: Invalid
				   ^^^^^^^

The last line with 'Invalid' should have printed the high level
MCE error type description we get from mce_severity, i.e.
something like:

[Hardware Error]: Machine check: Action required: data load error in a user process

this happens due to the fact that mce_no_way_out() iterates over
all MCA banks and possibly overwrites the @msg argument which is
used in the panic printing later.

Change behavior to take the message of only and the (last)
critical MCE it detects.

32935497ea KVM: nSVM: Check for NRIPS support before updating control field
commit f104765b4f upstream.

If hardware doesn't support DecodeAssist - a feature that provides
more information about the intercept in the VMCB, KVM decodes the
instruction and then updates the next_rip vmcb control field.
However, NRIP support itself depends on cpuid Fn8000_000A_EDX[NRIPS].
Since skip_emulated_instruction() doesn't verify nrip support
before accepting control.next_rip as valid, avoid writing this
field if support isn't present.

99355e1c8b MIPS: Fix enabling of DEBUG_STACKOVERFLOW
commit 5f35b9cd55 upstream.

Commit 334c86c494 ("MIPS: IRQ: Add stackoverflow detection") added
kernel stack overflow detection, however it only enabled it conditional
upon the preprocessor definition DEBUG_STACKOVERFLOW, which is never
actually defined. The Kconfig option is called DEBUG_STACKOVERFLOW,
which manifests to the preprocessor as CONFIG_DEBUG_STACKOVERFLOW, so
switch it to using that definition instead.

dd727f0191 x86/asm/irq: Stop relying on magic JMP behavior for early_idt_handlers
commit 425be5679f upstream.

The early_idt_handlers asm code generates an array of entry
points spaced nine bytes apart. It's not really clear from that
code or from the places that reference it what's going on, and
the code only works in the first place because GAS never
generates two-byte JMP instructions when jumping to global
labels.

Clean up the code to generate the correct array stride (member size)
explicitly. This should be considerably more robust against
screw-ups, as GAS will warn if a .fill directive has a negative
count. Using '. =' to advance would have been even more robust
(it would generate an actual error if it tried to move
backwards), but it would pad with nulls, confusing anyone who
tries to disassemble the code. The new scheme should be much
clearer to future readers.

While we're at it, improve the comments and rename the array and
common code.

Binutils may start relaxing jumps to non-weak labels. If so,
this change will fix our build, and we may need to backport this
change.

e0373a44e9 x86: bpf_jit: fix compilation of large bpf programs
commit 3f7352bf21 upstream.

x86 has variable length encoding. x86 JIT compiler is trying
to pick the shortest encoding for given bpf instruction.
While doing so the jump targets are changing, so JIT is doing
multiple passes over the program. Typical program needs 3 passes.
Some very short programs converge with 2 passes. Large programs
may need 4 or 5. But specially crafted bpf programs may hit the
pass limit and if the program converges on the last iteration
the JIT compiler will be producing an image full of 'int 3' insns.
Fix this corner case by doing final iteration over bpf program.

f6d58ed718 ARM: 8356/1: mm: handle non-pmd-aligned end of RAM
commit 965278dcb8 upstream.

At boot time we round the memblock limit down to section size in an
attempt to ensure that we will have mapped this RAM with section
mappings prior to allocating from it. When mapping RAM we iterate over
PMD-sized chunks, creating these section mappings.

Section mappings are only created when the end of a chunk is aligned to
section size. Unfortunately, with classic page tables (where PMD_SIZE is
2 * SECTION_SIZE) this means that if a chunk is between 1M and 2M in
size the first 1M will not be mapped despite having been accounted for
in the memblock limit. This has been observed to result in page tables
being allocated from unmapped memory, causing boot-time hangs.

This patch modifies the memblock limit rounding to always round down to
PMD_SIZE instead of SECTION_SIZE. For classic MMU this means that we
will round the memblock limit down to a 2M boundary, matching the limits
on section mappings, and preventing allocations from unmapped memory.
For LPAE there should be no change as PMD_SIZE == SECTION_SIZE.

112a154506 ARM: fix missing syscall trace exit
commit 1b97937246 upstream.

Josh Stone reports:

I've discovered a case where both arm and arm64 will miss a ptrace
syscall-exit that they should report. If the syscall is entered
without TIF_SYSCALL_TRACE set, then it goes on the fast path. It's
then possible to have TIF_SYSCALL_TRACE added in the middle of the
syscall, but ret_fast_syscall doesn't check this flag again.

Fix this by always checking for a syscall trace in the fast exit path.

3a5f810372 ARM: mvebu: do not register custom DMA operations when coherency is disabled
This patch is a partial backport of commit ef01c6c36b ("ARM: mvebu:
remove Armada 375 Z1 workaround for I/O coherency"). This commit was
merged in v3.19, so kernel versions later than v3.19 are not affected
by the problem that this commit fixes.

It does not make a lot of sense to backport this commit entirely,
since it is mainly removing some no longer useful code. However, this
commit is also making sure that the bus_register_notifier that
register the custom DMA operations that should be used for HW I/O
coherency does not get registered when said HW I/O coherency is not
enabled.

This is particularly critical since we have decided to disable HW I/O
coherency completely in all kernels < 4.0, to be on the safe side,
while experimenting a new implementation of the HW I/O coherency in >=
4.0.

Without this commit, kernels earlier than 3.18 have the custom DMA
operations normally used for HW I/O coherency registered (they don't
do cache maintenance operations), while HW I/O coherency is
disabled. It essentially causes every DMA transfer to transfer
garbage.

The issue fixed by this commit was introduced by 5ab5afd8ba ("ARM:
mvebu: implement Armada 375 coherency workaround"), but it was not
visible until now since it didn't cause any problem when HW I/O
coherency is enabled.

13242f9325 config: Enable NEED_DMA_MAP_STATE by default when SWIOTLB is selected
commit a6dfa128ce upstream.

A huge amount of NIC drivers use the DMA API, however if
compiled under 32-bit an very important part of the DMA API can
be ommitted leading to the drivers not working at all
(especially if used with 'swiotlb=force iommu=soft').

As Prashant Sreedharan explains it: "the driver [tg3] uses
DEFINE_DMA_UNMAP_ADDR(), dma_unmap_addr_set() to keep a copy of
the dma "mapping" and dma_unmap_addr() to get the "mapping"
value. On most of the platforms this is a no-op, but ... with
"iommu=soft and swiotlb=force" this house keeping is required,
... otherwise we pass 0 while calling pci_unmap_/pci_dma_sync_
instead of the DMA address."

As such enable this even when using 32-bit kernels.

0df938e720 arm64: add missing PAGE_ALIGN() to __dma_free()
commit 2cff98b99c upstream.

__dma_alloc() does a PAGE_ALIGN() on the passed in size argument before
doing anything else. __dma_free() does not. And because it doesn't, it is
possible to leak memory should size not be an integer multiple of PAGE_SIZE.

The solution is to add a PAGE_ALIGN() to __dma_free() like is done in
__dma_alloc().

Additionally, this patch removes a redundant PAGE_ALIGN() from
__dma_alloc_coherent(), since __dma_alloc_coherent() can only be called
from __dma_alloc(), which already does a PAGE_ALIGN() before the call.

441b34775e parisc, metag: Fix crashes due to stack randomization on stack-grows-upwards architectures
commit d045c77c1a upstream.

On architectures where the stack grows upwards (CONFIG_STACK_GROWSUP=y,
currently parisc and metag only) stack randomization sometimes leads to crashes
when the stack ulimit is set to lower values than STACK_RND_MASK (which is 8 MB
by default if not defined in arch-specific headers).

The problem is, that when the stack vm_area_struct is set up in fs/exec.c, the
additional space needed for the stack randomization (as defined by the value of
STACK_RND_MASK) was not taken into account yet and as such, when the stack
randomization code added a random offset to the stack start, the stack
effectively got smaller than what the user defined via rlimit_max(RLIMIT_STACK)
which then sometimes leads to out-of-stack situations and crashes.

This patch fixes it by adding the maximum possible amount of memory (based on
STACK_RND_MASK) which theoretically could be added by the stack randomization
code to the initial stack size. That way, the user-defined stack size is always
guaranteed to be at minimum what is defined via rlimit_max(RLIMIT_STACK).

This bug is currently not visible on the metag architecture, because on metag
STACK_RND_MASK is defined to 0 which effectively disables stack randomization.

The changes to fs/exec.c are inside an "#ifdef CONFIG_STACK_GROWSUP"
section, so it does not affect other platformws beside those where the
stack grows upwards (parisc and metag).

commit d6c763afab upstream.

Since commit 8a0a9bd4db ('random: make get_random_int() more

so comment and hack introduced in mmap_rnd() as part of commit
1d18c47c73 ('arm64: MMU fault handling and page table management')
are incorrects.

Commit 1d18c47c73 seems to use the same hack introduced by
commit a5adc91a4b ('powerpc: Ensure random space between stack
and mmaps'), latter copied in commit 5a0efea09f ('sparc64: Sharpen
address space randomization calculations.').

But both architectures were cleaned up as part of commit
fa8cbaaf5a ('powerpc+sparc64/mm: Remove hack in mmap randomize
layout') as hack is no more needed since commit 8a0a9bd4db.

So the present patch removes the comment and the hack around
get_random_int() on AArch64's mmap_rnd().

8baeb01f38 ARM: dts: imx23-olinuxino: Fix dr_mode of usb0
commit 0fdebe1a2f upstream.

The dr_mode of usb0 on imx233-olinuxino is left to default "otg".
Since the green LED (GPIO2_1) on imx233-olinuxino is connected to the
same pin as USB_OTG_ID it's possible to disable USB host by LED toggling:

echo 0 > /sys/class/leds/green/brightness
[ 1068.890000] ci_hdrc ci_hdrc.0: remove, state 1
[ 1068.890000] usb usb1: USB disconnect, device number 1

[ 1074.460000] ci_hdrc ci_hdrc.0: timeout waiting for 00000800 in 11

This patch fixes the issue by setting dr_mode to "host" in the dts file.

f2b73ee267 x86/efi: Store upper bits of command line buffer address in ext_cmd_line_ptr
commit 98b228f550 upstream.

Until now, the EFI stub was only setting the 32 bit cmd_line_ptr in
the setup_header structure, so on 64 bit platforms this could be truncated.
This patch adds setting the upper bits of the buffer address in
ext_cmd_line_ptr. This case was likely never hit, as the allocation
for this buffer is done at the lowest available address. Only
x86_64 kernels have this problem, as the 1-1 mapping mandated
by EFI ensures that all memory is 32 bit addressable on 32 bit
platforms. The EFI stub does not support mixed mode, so the
32 bit kernel on 64 bit firmware case does not need to be handled.

db69415c3f arm64: KVM: Do not use pgd_index to index stage-2 pgd
commit 04b8dc85bf upstream.

The kernel's pgd_index macro is designed to index a normal, page
sized array. KVM is a bit diffferent, as we can use concatenated
pages to have a bigger address space (for example 40bit IPA with
4kB pages gives us an 8kB PGD.

In the above case, the use of pgd_index will always return an index
inside the first 4kB, which makes a guest that has memory above
0x8000000000 rather unhappy, as it spins forever in a page fault,
whist the host happilly corrupts the lower pgd.

The obvious fix is to get our own kvm_pgd_index that does the right
thing(tm).

Tested on X-Gene with a hacked kvmtool that put memory at a stupidly
high address.

026f44213a arm64: KVM: Fix HCR setting for 32bit guests
commit 801f6772ce upstream.

Commit b856a59141 (arm/arm64: KVM: Reset the HCR on each vcpu
when resetting the vcpu) moved the init of the HCR register to
happen later in the init of a vcpu, but left out the fixup
done in kvm_reset_vcpu when preparing for a 32bit guest.

As a result, the 32bit guest is run as a 64bit guest, but the
rest of the kernel still manages it as a 32bit. Fun follows.

Moving the fixup to vcpu_reset_hcr solves the problem for good.

b1ad37aa9e arm/arm64: KVM: Require in-kernel vgic for the arch timers
commit 05971120fc upstream.

It is curently possible to run a VM with architected timers support
without creating an in-kernel VGIC, which will result in interrupts from
the virtual timer going nowhere.

To address this issue, move the architected timers initialization to the
time when we run a VCPU for the first time, and then only initialize
(and enable) the architected timers if we have a properly created and
initialized in-kernel VGIC.

When injecting interrupts from the virtual timer to the vgic, the
current setup should ensure that this never calls an on-demand init of
the VGIC, which is the only call path that could return an error from
kvm_vgic_inject_irq(), so capture the return value and raise a warning
if there's an error there.

We also change the kvm_timer_init() function from returning an int to be
a void function, since the function always succeeds.

d901bcf087 arm/arm64: KVM: Introduce stage2_unmap_vm
commit 957db105c9 upstream.

Introduce a new function to unmap user RAM regions in the stage2 page
tables. This is needed on reboot (or when the guest turns off the MMU)
to ensure we fault in pages again and make the dcache, RAM, and icache
coherent.

Using unmap_stage2_range for the whole guest physical range does not
work, because that unmaps IO regions (such as the GIC) which will not be
recreated or in the best case faulted in on a page-by-page basis.

Call this function on secondary and subsequent calls to the
KVM_ARM_VCPU_INIT ioctl so that a reset VCPU will detect the guest
Stage-1 MMU is off when faulting in pages and make the caches coherent.
