sknr d4c523ed5c fix(validate): enforce trust ordering, add opt-in --stale gate, and harden legacy checks 
- gate superseded verifications (verified.at < generated.at) under --strict
- add opt-in --stale flag to validate for explicit lifecycle gating
- emit concept-specific warnings when stale_after date is reached
- isolate v0.1 legacy checks to frontmatter (timestamp:) and unfenced body (# Citations)
- expose optional stale parameter in okf_validate MCP tool
- align mock timestamps in benchmarks/data/knowledge fixtures
- add unit tests for trust ordering, stale gating, and legacy checks
