3e921b4571 crypto: s390/ghash - Fix incorrect ghash icv buffer handling.
commit a1cae34e23 upstream.

Multitheaded tests showed that the icv buffer in the current ghash
implementation is not handled correctly. A move of this working ghash
buffer value to the descriptor context fixed this. Code is tested and
verified with an multithreaded application via af_alg interface.

5d32cdce01 s390/hibernate: fix save and restore of kernel text section
commit d744194956 upstream.

Sebastian reported a crash caused by a jump label mismatch after resume.
This happens because we do not save the kernel text section during suspend
and therefore also do not restore it during resume, but use the kernel image
that restores the old system.

This means that after a suspend/resume cycle we lost all modifications done
to the kernel text section.
The reason for this is the pfn_is_nosave() function, which incorrectly
returns that read-only pages don't need to be saved. This is incorrect since
we mark the kernel text section read-only.
We still need to make sure to not save and restore pages contained within
NSS and DCSS segment.
To fix this add an extra case for the kernel text section and only save
those pages if they are not contained within an NSS segment.

Fixes the following crash (and the above bugs as well):

Jump label code mismatch at netif_receive_skb_internal+0x28/0xd0

cde78e45e7 KVM: s390: reinjection of irqs can fail in the tpi handler
commit 15462e37ca upstream.

The reinjection of an I/O interrupt can fail if the list is at the limit
and between the dequeue and the reinjection, another I/O interrupt is
injected (e.g. if user space floods kvm with I/O interrupts).

This patch avoids this memory leak and returns -EFAULT in this special
case. This error is not recoverable, so let's fail hard. This can later
be avoided by not dequeuing the interrupt but working directly on the
locked list.

3ac8b6fa58 KVM: s390: fix handling of write errors in the tpi handler
commit 261520dcfc upstream.

If the I/O interrupt could not be written to the guest provided
area (e.g. access exception), a program exception was injected into the
guest but "inti" wasn't freed, therefore resulting in a memory leak.

In addition, the I/O interrupt wasn't reinjected. Therefore the dequeued
interrupt is lost.

This patch fixes the problem while cleaning up the function and making the
cc and rc logic easier to handle.

903575f1b0 vm: add VM_FAULT_SIGSEGV handling support
commit 33692f2759 upstream.

The core VM already knows about VM_FAULT_SIGBUS, but cannot return a
"you should SIGSEGV" error, because the SIGSEGV case was generally
handled by the caller - usually the architecture fault handler.

That results in lots of duplication - all the architecture fault
handlers end up doing very similar "look up vma, check permissions, do
retries etc" - but it generally works. However, there are cases where
the VM actually wants to SIGSEGV, and applications _expect_ SIGSEGV.

In particular, when accessing the stack guard page, libsigsegv expects a
SIGSEGV. And it usually got one, because the stack growth is handled by
that duplicated architecture fault handler.

However, when the generic VM layer started propagating the error return
from the stack expansion in commit fee7e49d45 ("mm: propagate error
from stack expansion even for guard page"), that now exposed the
existing VM_FAULT_SIGBUS result to user space. And user space really
expected SIGSEGV, not SIGBUS.

To fix that case, we need to add a VM_FAULT_SIGSEGV, and teach all those
duplicate architecture fault handlers about it. They all already have
the code to handle SIGSEGV, so it's about just tying that new return
value to the existing code, but it's all a bit annoying.

This is the mindless minimal patch to do this. A more extensive patch
would be to try to gather up the mostly shared fault handling logic into
one generic helper routine, and long-term we really should do that
cleanup.

Just from this patch, you can generally see that most architectures just
copied (directly or indirectly) the old x86 way of doing things, but in
the meantime that original x86 model has been improved to hold the VM
semaphore for shorter times etc and to handle VM_FAULT_RETRY and other
"newer" things, so it would be a good idea to bring all those
improvements to the generic case and teach other architectures about
them too.

69ba3b2531 KVM: s390: Fix ipte locking
commit 1365039d0c upstream.

ipte_unlock_siif uses cmpxchg to replace the in-memory data of the ipte

union ipte_control {
        unsigned long val;
        struct {
                unsigned long k  : 1;
                unsigned long kh : 31;
                unsigned long kg : 32;
        };
};
[...]
static void ipte_unlock_siif(struct kvm_vcpu *vcpu)
{
        union ipte_control old, new, *ic;

        ic = &vcpu->kvm->arch.sca->ipte_control;
        do {
                new = old = ACCESS_ONCE(*ic);
                new.kh--;
                if (!new.kh)
                        new.k = 0;
        } while (cmpxchg(&ic->val, old.val, new.val) != old.val);
        if (!new.kh)
                wake_up(&vcpu->kvm->arch.ipte_wq);
}

The new value, is loaded twice from memory with gcc 4.7.2 of
fedora 18, despite the ACCESS_ONCE:

--->

If the memory value changes between the first load (l) and the second
load (lg) we are broken. If that happens VCPU threads will hang
(unkillable) in handle_ipte_interlock.

Andreas Krebbel analyzed this and tracked it down to a compiler bug in
that version:
"while it is not that obvious the C99 standard basically forbids
duplicating the memory access also in that case. For an argumentation of
a similiar case please see:
https://gcc.gnu.org/bugzilla/show_bug.cgi?id=22278#c43

For the implementation-defined cases regarding volatile there are some
GCC-specific clarifications which can be found here:
https://gcc.gnu.org/onlinedocs/gcc/Volatiles.html#Volatiles

I've tracked down the problem with a reduced testcase. The problem was
that during a tree level optimization (SRA - scalar replacement of
aggregates) the volatile marker is lost. And an RTL level optimizer (CSE
- common subexpression elimination) then propagated the memory read into
  its second use introducing another access to the memory location. So
indeed Christian's suspicion that the union access has something to do
with it is correct (since it triggered the SRA optimization).

This issue has been reported and fixed in the GCC 4.8 development cycle:
https://gcc.gnu.org/bugzilla/show_bug.cgi?id=58145"

This patch replaces the ACCESS_ONCE scheme with a barrier() based scheme
that should work for all supported compilers.

The PSW mask check of the PTRACE_POKEUSR_AREA command is incorrect.
The PSW_MASK_USER define contains the PSW_MASK_ASC bits, the ptrace
interface accepts all combinations for the address-space-control
bits. To protect the kernel space the PSW mask check in ptrace needs
to reject the address-space-control bit combination for home space.

8fb878c5f1 s390/MSI: Use standard mask and unmask funtions
MSI irqchip in s390 has its own mask and unmask MSI irq
functions, zpci_enable_irq() and zpci_disable_irq().
They mask and unmask MSI irq in standard ways, no arch
special. MSI driver provides two global standard functions
mask_msi_irq() and unmask_msi_irq(). Local zpci_enable_irq()
and zpci_disable_irq() are almost the same as the standard
two. the difference is local mask/unmask functions
read the mask status before mask and unmask everytime.
Then change the value and rewrite to hardware. In standard
functions, save the mask status after mask and unmask msi
irq, and use the cached status to change the mask status.
When we mask or unmask a MSI irq, we always cache its
mask status except we know need not to cache it, like in
pci_msi_shutdown. So use the standard functions to replace
the local is safe.

d09a08e030 KVM: s390: add sie.h uapi header file to Kbuild and remove header dependency
sie.h was missing in arch/s390/include/uapi/asm/Kbuild and therefore missed
the "make headers_check" target.
If added it reveals that also arch/s390/include/asm/sigp.h would become uapi.
This is something we certainly do not want. So remove that dependency as well.

The header file was merged with ceae283bb2 "KVM: s390: add sie exit
reasons tables", therefore we never had a kernel release with this commit and
can still change anything.

b9c9a33b76 s390/compat: correct ucontext layout for high gprs
The uc_sigmask definition in the kernel differs from the one in the
glibc, the kernel uc_sigmask has 64 bits while the glibc verison
is 1024 bits. The extension of the ucontext structure for 64-bit
register support for 31-bit compat processes added a new field
uc_gprs_high which starts 8 bytes after the uc_sigmask field.
As the glibc view of the ucontext assumes a size of 128 bytes for
uc_sigmask add a 120 byte padding to the kernel structure
ucontext_extended after the 8 byte uc_sigmask.

f8b1350560 s390/uaccess: always load the kernel ASCE after task switch
This patch fixes a problem introduced with git commit beef560b4c
"s390/uaccess: simplify control register updates".

The switch_mm function is not called if the next process is a kernel
thread without an attached mm or is a nop if the mm does not change.
But CR1 still needs to be loaded with the kernel ASCE in case the
code returns to a uaccess function that uses the secondary space mode.

In addition move the set_fs call from finish_arch_switch to
finish_arch_post_lock_switch and then remove finish_arch_switch.

c177c81e09 hugetlb: restrict hugepage_migration_support() to x86_64
Currently hugepage migration is available for all archs which support
pmd-level hugepage, but testing is done only for x86_64 and there're
bugs for other archs. So to avoid breaking such archs, this patch
limits the availability strictly to x86_64 until developers of other
archs get interested in enabling this feature.

Simply disabling hugepage migration on non-x86_64 archs is not enough to
fix the reported problem where sys_move_pages() hits the BUG_ON() in
follow_page(FOLL_GET), so let's fix this by checking if hugepage
migration is supported in vma_migratable().

Pull first set of s390 updates from Martin Schwidefsky:
 "The biggest change in this patchset is conversion from the bootmem
  bitmaps to the memblock code. This conversion requires two common
  code patches to introduce the 'physmem' memblock list.

  We experimented with ticket spinlocks but in the end decided against
  them as they perform poorly on virtualized systems. But the spinlock
  cleanup and some small improvements are included.

  The uaccess code got another optimization, the get_user/put_user calls
  are now inline again for kernel compiles targeted at z10 or newer
  machines. This makes the text segment shorter and the code gets a
  little bit faster.

  And as always some bug fixes"

3480593131 net: filter: get rid of BPF_S_* enum
This patch finally allows us to get rid of the BPF_S_* enum.
Currently, the code performs unnecessary encode and decode
workarounds in seccomp and filter migration itself when a filter
is being attached in order to overcome BPF_S_* encoding which
is not used anymore by the new interpreter resp. JIT compilers.

Keeping it around would mean that also in future we would need
to extend and maintain this enum and related encoders/decoders.
We can get rid of all that and save us these operations during
filter attaching. Naturally, also JIT compilers need to be updated
by this.

Before JIT conversion is being done, each compiler checks if A
is being loaded at startup to obtain information if it needs to
emit instructions to clear A first. Since BPF extensions are a
subset of BPF_LD | BPF_{W,H,B} | BPF_ABS variants, case statements
for extensions can be removed at that point. To ease and minimalize
code changes in the classic JITs, we have introduced bpf_anc_helper().

Tested with test_bpf on x86_64 (JIT, int), s390x (JIT, int),
arm (JIT, int), i368 (int), ppc64 (JIT, int); for sparc we
unfortunately didn't have access, but changes are analogous to
the rest.

5a5e65361f KVM: s390: Intercept the tprot instruction
Based on original patch from Jeng-fang (Nick) Wang

When standby memory is specified for a guest Linux, but no virtual memory has
been allocated on the Qemu host backing that guest, the guest memory detection
process encounters a memory access exception which is not thrown from the KVM
handle_tprot() instruction-handler function. The access exception comes from
sie64a returning EFAULT, which then passes an addressing exception to the guest.
Unfortunately this does not the proper PSW fixup (nullifying vs.
suppressing) so the guest will get a fault for the wrong address.

Let's just intercept the tprot instruction all the time to do the right thing
and not go the page fault handler path for standby memory. tprot is only used
by Linux during startup so some exits should be ok.
Without this patch, standby memory cannot be used with KVM.

3192c63950 KVM: s390: a VCPU is already started when delivering interrupts
This patch removes the start of a VCPU when delivering a RESTART interrupt.
Interrupt delivery is called from kvm_arch_vcpu_ioctl_run. So the VCPU is
already considered started - no need to call kvm_s390_vcpu_start. This function
will early exit anyway.

2de3bfc25a KVM: s390: check the given debug flags, not the set ones
This patch fixes a minor bug when updating the guest debug settings.
We should check the given debug flags, not the already set ones.
Doesn't do any harm but too many (for now unused) flags could be set internally
without error.

22ff4a3366 KVM: s390: clean up interrupt injection in sigp code
We have all the logic to inject interrupts available in
kvm_s390_inject_vcpu(), so let's use it instead of
injecting irqs manually to the list in sigp code.

SIGP stop is special because we have to check the
action_flags before injecting the interrupt. As
the action_flags are not available in kvm_s390_inject_vcpu()
we leave the code for the stop order code untouched for now.
