a6889df540 mmc: block: fix packed command header endianness
commit f68381a70b upstream.

The code that fills packed command header assumes that CPU runs in
little-endian mode. Hence the header is malformed in big-endian mode
and causes MMC data transfer errors:

[  563.200828] mmcblk0: error -110 transferring data, sector 2048, nr 8, cmd response 0x900, card status 0xc40
[  563.219647] mmcblk0: packed cmd failed, nr 2, sectors 16, failure index: -1

Convert header data to LE.

a888f891c2 mmc: longer timeout for long read time quirk
commit 32ecd320db upstream.

008GE0 Toshiba mmc in some Intel Baytrail tablets responds to
MMC_SEND_EXT_CSD in 450-600ms.

This patch will...

() Increase the long read time quirk timeout from 300ms to 600ms. Original
   author of that quirk says 300ms was only a guess and that the number
   may need to be raised in the future.

c47ae034bb mmc: mmc: Fix partition switch timeout for some eMMCs
commit 1c447116d0 upstream.

Some eMMCs set the partition switch timeout too low.

Now typically eMMCs are considered a critical component (e.g. because
they store the root file system) and consequently are expected to be
reliable. Thus we can neglect the use case where eMMCs can't switch
reliably and we might want a lower timeout to facilitate speedy
recovery.

Although we could employ a quirk for the cards that are affected (if
we could identify them all), as described above, there is little
benefit to having a low timeout, so instead simply set a minimum
timeout.

The minimum is set to 300ms somewhat arbitrarily - the examples that
have been seen had a timeout of 10ms but were sometimes taking 60-70ms.

6a4c189fc3 mmc: sdhci: fix data timeout (part 2)
commit 7f05538af7 upstream.

The calculation for the timeout based on the number of card clocks is
incorrect.  The calculation assumed:

	timeout in microseconds = clock cycles / clock in Hz

which is clearly a several orders of magnitude wrong. Fix this by
multiplying the clock cycles by 1000000 prior to dividing by the Hz
based clock. Also, as per part 1, ensure that the division rounds
up.

As this needs 64-bit math via do_div(), avoid it if the clock cycles
is zero.

cc4cfd6022 mmc: sdhci: fix data timeout (part 1)
commit fafcfda9e7 upstream.

The data timeout gives the minimum amount of time that should be
waited before timing out if no data is received from the card.
Simply dividing the nanosecond part by 1000 does not give this
required guarantee, since such a division rounds down. Use
DIV_ROUND_UP() to give the desired timeout.

6f40d49618 mmc: sd: limit SD card power limit according to cards capabilities
commit d9812780a0 upstream.

The SD card specification allows cards to error out a SWITCH command
where the requested function in a group is not supported. The spec
provides for a set of capabilities which indicate which functions are
supported.

In the case of the power limit, requesting an unsupported power level
via the SWITCH command fails, resulting in the power level remaining at
the power-on default of 0.72W, even though the host and card may support
higher powers levels.

This has been seen with SanDisk 8GB cards, which support the default
0.72W and 1.44W (200mA and 400mA) in combination with an iMX6 host,
supporting up to 2.88W (800mA). This currently causes us to try to set
a power limit function value of '3' (2.88W) which the card errors out
on, and thereby causes the power level to remain at 0.72W rather than
the desired 1.44W.

Arrange to limit the selected current limit by the capabilities reported
by the card to avoid the SWITCH command failing. Select the highest
current limit that the host and card combination support.

c35e02fb10 mmc: sdhci: Fix sdhci_runtime_pm_bus_on/off()
commit 5c671c410c upstream.

sdhci has a legacy facility to prevent runtime suspend if the
bus power is on. This is needed in cases where the power to
the card is dependent on the bus power. It is controlled by
a pair of functions: sdhci_runtime_pm_bus_on() and
sdhci_runtime_pm_bus_off(). These functions use a boolean
variable 'bus_on' to ensure changes are always paired.
There is an additional check for 'runtime_suspended' which is
the problem. In fact, its use is ill-conceived as the only
requirement for the logic is that 'on' and 'off' are paired,
which is actually broken by the check, for example if the bus
power is turned on during runtime resume. So remove the check.

926c0116ab mmc: sdio: Fix invalid vdd in voltage switch power cycle
commit d9bfbb95ed upstream.

The 'ocr' parameter passed to mmc_set_signal_voltage()
defines the power-on voltage used when power cycling
after a failure to set the voltage. However, in the
case of mmc_sdio_init_card(), the value passed has the
R4_18V_PRESENT flag set which is not valid for power-on
and results in an invalid vdd. Fix by passing the card's
ocr value which does not have the flag.

131de5ec6f mmc: remove bondage between REQ_META and reliable write
commit d3df0465db upstream.

Anytime a write operation is performed with Reliable Write flag enabled,
the eMMC device is enforced to bypass the cache and do a write to the
underling NVM device by Jedec specification; this causes a performance
penalty since write operations can't be optimized by the device cache.

In our tests, we replayed a typical mobile daily trace pattern and found
~9% overall time reduction in trace replay by using this patch. Also the
write ops within 4KB~64KB chunk size range get a 40~60% performance
improvement by using the patch (as this range of write chunks are the ones
affected by REQ_META).

This patch has been discussed in the Mobile & Embedded Linux Storage Forum
and it's the results of feedbacks from many people. We also checked with
fsdevl and f2fs mailing list developers that this change in the usage of
REQ_META is not affecting FS behavior and we got positive feedbacks.
Reporting here the feedbacks:

Because in SMP, "mrq" has race condition between below two paths:
path1: CPU0: <tasklet context>
  static void mmc_wait_data_done(struct mmc_request *mrq)
  {
     mrq->host->context_info.is_done_rcv = true;
     //
     // If CPU0 has just finished "is_done_rcv = true" in path1, and at
     // this moment, IRQ or ICache line missing happens in CPU0.
     // What happens in CPU1 (path2)?
     //
     // If the mmcqd thread in CPU1(path2) hasn't entered to sleep mode:
     // path2 would have chance to break from wait_event_interruptible
     // in mmc_wait_for_data_req_done and continue to run for next
     // mmc_request (mmc_blk_rw_rq_prep).
     //
     // Within mmc_blk_rq_prep, mrq is cleared to 0.
     // If below line still gets host from "mrq" as the result of
     // compiler, the panic happens as we traced.
     wake_up_interruptible(&mrq->host->context_info.wait);
  }

path2: CPU1: <The mmcqd thread runs mmc_queue_thread>
  static int mmc_wait_for_data_req_done(...
  {
     ...
     while (1) {
           wait_event_interruptible(context_info->wait,
                   (context_info->is_done_rcv ||
                    context_info->is_new_req));
     	   static void mmc_blk_rw_rq_prep(...
           {
           ...
           memset(brq, 0, sizeof(struct mmc_blk_request));

This issue happens very coincidentally; however adding mdelay(1) in
mmc_wait_data_done as below could duplicate it easily.

   static void mmc_wait_data_done(struct mmc_request *mrq)
   {
     mrq->host->context_info.is_done_rcv = true;
+    mdelay(1);
     wake_up_interruptible(&mrq->host->context_info.wait);
    }

At runtime, IRQ or ICache line missing may just happen at the same place
of the mdelay(1).

This patch gets the mmc_context_info at the beginning of function, it can
avoid this race condition.

725614a05d mmc: sdhci: Fix FSL ESDHC reset handling quirk
commit 304f0a98d1 upstream.

The commit 0718e59ae2 ("mmc: sdhci: move FSL ESDHC reset handling quirk into

the interrupt registers restored after a reset. So it moves
SDHCI_QUIRK_RESTORE_IRQS_AFTER_RESET quirk handling code into the
esdhc-imx driver only. Unfortunately the same controller is used in
other boards which use the of-esdhc driver instead (like powerpc P2020).

Because 'end_trans' is never set, omap_hsmmc_xfer_done() is never invoked
making core layer not to be aware of DTO/DCRC errors. Because of this
any command invoked after DTO/DCRC error leads to a hang.

The current handler of MMC_BLK_CMD_ERR in mmc_blk_issue_rw_rq function

The problem scenario is as follows:
(1) Request A is ongoing;
(2) Request B arrived, and finally mmc_blk_issue_rw_rq() is called;
(3) Request A encounters the MMC_BLK_CMD_ERR error;
(4) In the error handling of MMC_BLK_CMD_ERR, suppose mmc_blk_cmd_err()
    end request A completed and return zero. Continue the error handling,
    suppose mmc_blk_reset() reset device success;
(5) Continue the execution, while loop completed because variable ret
    is zero now;
(6) Finally, mmc_blk_issue_rw_rq() return without processing request B.

The process related to the missing request may wait that IO request
complete forever, possibly crashing the application or hanging the system.

cfe3f61153 mmc: sh_mmcif: Fix timeout value for command request
commit bad4371d87 upstream.

f9fd54f22e ("mmc: sh_mmcif: Use msecs_to_jiffies() for host->timeout")
changed the timeout value from 1000 jiffies to 1s. In the case where
HZ is 1000 the values are the same. However, for smaller HZ values the
timeout is now smaller, 1s instead of 10s in the case of HZ=100.

Since the timeout occurs in spite of a normal data transfer a timeout of
10s seems more appropriate. This restores the previous timeout in the
case where HZ=100 and results in an increase over the previous timeout
for larger values of HZ.

a66664cc85 mmc: sdhci-pxav3: Fix Armada 38x controller's caps according to erratum ERR-7878951
commit a39128bcd6 upstream.

According to erratum 'ERR-7878951' Armada 38x SDHCI controller has
different capabilities than the ones shown in its registers:

- it doesn't support the voltage switching: it can work either with

- it doesn't support the SDR104 mode
- SDR50 mode doesn't need tuning

The SDHCI_QUIRK_MISSING_CAPS quirk is used for updating the
capabilities accordingly.

2234e863e9 mmc: sdhci-pxav3: fix setting of pdata->clk_delay_cycles
commit 14460dbaf7 upstream.

Current code checks "clk_delay_cycles > 0" to know whether the optional

44baa458dc mmc: sdhci-pxav3: fix race between runtime pm and irq
commit 3bb10f6093 upstream.

This patch is to fix a race condition that may cause an unhandled irq,
which results in big sdhci interrupt numbers and endless "mmc1: got irq
while runtime suspended" msgs before v3.15.

Consider following scenario:

      CPU0                            CPU1
                              sdhci_pxav3_runtime_suspend()
                               spin_lock_irqsave(&host->lock, flags);
 sdhci_irq()
  spining on the &host->lock
                               host->runtime_suspended = true;
                               spin_unlock_irqrestore(&host->lock, flags);
  get the &host->lock
  runtime_suspended is true now
  return IRQ_NONE;

Fix this race by using the core sdhci.c supplied sdhci_runtime_suspend_host()
in runtime suspend hook which will disable card interrupts. We also use the
sdhci_runtime_resume_host() in the runtime resume hook accordingly.

a5c1132309 mmc: sdhci-pxav3: fix unbalanced clock issues during probe
commit 62cf983ad8 upstream.

Commit 0dcaa2499b ("sdhci-pxav3: Fix runtime PM initialization") tries
to fix one hang issue caused by calling sdhci_add_host() on a suspended
device. The fix enables the clock twice, once by clk_prepare_enable() and
another by pm_runtime_get_sync(), meaning that the clock will never be
gated at runtime PM suspend. I observed the power consumption regression on
Marvell BG2Q SoCs.

In fact, the fix is not correct. There still be a very small window
during which a runtime suspend might somehow occur after pm_runtime_enable()
but before pm_runtime_get_sync().

This patch fixes all of the two problems by just incrementing the usage
counter before pm_runtime_enable(). It also adjust the order of disabling
runtime pm and storing the usage count in the error path to handle clock
gating properly.

462dd2f906 mmc: sdhci: Fix sleep in atomic after inserting SD card
commit 2836766a9d upstream.

Sleep in atomic context happened on Trats2 board after inserting or
removing SD card because mmc_gpio_get_cd() was called under spin lock.

Fix this by moving card detection earlier, before acquiring spin lock.
The mmc_gpio_get_cd() call does not have to be protected by spin lock
because it does not access any sdhci internal data.
The sdhci_do_get_cd() call access host flags (SDHCI_DEVICE_DEAD). After
moving it out side of spin lock it could theoretically race with driver
removal but still there is no actual protection against manual card
eject.

d521c37bb5 mmc: sdhci-pxav3: do the mbus window configuration after enabling clocks
commit aa8165f914 upstream.

In commit 5491ce3f79 ("mmc: sdhci-pxav3: add support for the Armada
38x SDHCI controller"), the sdhci-pxav3 driver was extended to include
support for the SDHCI controller found in the Armada 38x
processor. This mainly involved adding some MBus window related
configuration.

However, this configuration is currently done too early in ->probe():
it is done before clocks are enabled, while this configuration
involves touching the registers of the controller, which will hang the
SoC if the clock is disabled. It wasn't noticed until now because the
bootloader typically leaves gatable clocks enabled, but in situations
where we have a deferred probe (due to a CD GPIO that cannot be taken,
for example), then the probe will be re-tried later, after a clock
disable has been done in the exit path of the failed probe attempt of
the device. This second probe() will hang the system due to the clock
being disabled.

This can for example be produced on Armada 385 GP, which has a CD GPIO
connected to an I2C PCA9555. If the driver for the PCA9555 is not
compiled into the kernel, then we will have the following sequence of
events:

  1. The SDHCI probes
  2. It does the MBus configuration (which works, because the clock is
     left enabled by the bootloader)
  3. It enables the clock
  4. It tries to get the CD GPIO, which fails due to the driver being
     missing, so -EPROBE_DEFER is returned.
  5. Before returning -EPROBE_DEFER, the driver cleans up what was
     done, which includes disabling the clock.
  6. Later on, the SDHCI probe is tried again.
  7. It does the MBus configuration, which hangs because the clock is
     no longer enabled.

This commit does the obvious fix of doing the MBus configuration after
the clock has been enabled by the driver.

dw_mci_ctrl_rd_thld() with an unconditional write to the CDTHRCTL
register at offset 0x100. However before version 240a, the FIFO region
started at 0x100, so the write messes with the FIFO and completely
breaks the driver.

If the version id < 240A, return early from dw_mci_ctl_rd_thld() so as
not to hit this problem.

aadefebf34 mmc: sdhci-s3c: fix runtime PM handling on sdhci_add_host() failure
commit 221414db19 upstream.

Runtime Power Management handling for the sdhci_add_host() failure
case in sdhci_s3c_probe() should match the code in sdhci_s3c_remove()
(which uses pm_runtime_disable() call which matches the earlier
pm_runtime_enable() one).  Fix it.

This patch fixes "BUG: spinlock bad magic on CPU#0, swapper/0/1" and
"Unbalanced pm_runtime_enable!" warnings.

ecca7685fd mmc: rtsx_pci_sdmmc: fix incorrect last byte in R2 response
commit d1419d50c1 upstream.

Current code erroneously fill the last byte of R2 response with an undefined
value. In addition, the controller actually 'offloads' the last byte
(CRC7, end bit) while receiving R2 response and thus it's impossible to get the
actual value. This could cause mmc stack to obtain inconsistent CID from the
same card after resume and misidentify it as a different card.

Fix by assigning dummy CRC and end bit: {7'b0, 1} = 0x1 to the last byte of R2.

01e9cd95ee mmc: don't request CD IRQ until mmc_start_host()
commit d4d1144908 upstream.

As soon as the CD IRQ is requested, it can trigger, since it's an
externally controlled event. If it does, delayed_work host->detect will
be scheduled.

Many host controller probe()s are roughly structured as:

*_probe() {
    host = sdhci_pltfm_init();
    mmc_of_parse(host->mmc);
    rc = sdhci_add_host(host);
    if (rc) {
        sdhci_pltfm_free();
        return rc;
    }

In 3.17, CD IRQs can are enabled quite early via *_probe() ->
mmc_of_parse() -> mmc_gpio_request_cd() -> mmc_gpiod_request_cd_irq().

Note that in linux-next, mmc_of_parse() calls mmc_gpio*d*_request_cd()
rather than mmc_gpio_request_cd(), and mmc_gpio*d*_request_cd() doesn't
call mmc_gpiod_request_cd_irq(). However, this issue still exists if
mmc_gpio_request_cd() is called directly before mmc_start_host().

sdhci_add_host() may fail part way through (e.g. due to deferred
probe for a vmmc regulator), and sdhci_pltfm_free() does nothing to
unrequest the CD IRQ nor cancel the delayed_work. sdhci_pltfm_free() is
coded to assume that if sdhci_add_host() failed, then the delayed_work
cannot (or should not) have been triggered.

This can lead to the following with CONFIG_DEBUG_OBJECTS_* enabled, when
kfree(host) is eventually called inside sdhci_pltfm_free():

WARNING: CPU: 2 PID: 6 at lib/debugobjects.c:263 debug_print_object+0x8c/0xb4()
ODEBUG: free active (active state 0) object type: timer_list hint: delayed_work_timer_fn+0x0/0x18

The object being complained about is host->detect.

There's no need to request the CD IRQ so early; mmc_start_host() already
requests it. For most SDHCI hosts at least, the typical call path that
does this is: *_probe() -> sdhci_add_host() -> mmc_add_host() ->
mmc_start_host(). Therefore, remove the call to mmc_gpiod_request_cd_irq()
from mmc_gpio_request_cd(). This also matches mmc_gpio*d*_request_cd(),
which already doesn't call mmc_gpiod_request_cd_irq().

However, some host controller drivers call mmc_gpio_request_cd() after
mmc_start_host() has already been called, and assume that this will also
call mmc_gpiod_request_cd_irq(). Update those drivers to explicitly call
mmc_gpiod_request_cd_irq() themselves. Ideally, these drivers should be
modified to move their call to mmc_gpio_request_cd() before their call
to mmc_add_host(). However that's too large a change for stable.

This solves the problem (eliminates the kernel error message above),
since it guarantees that the IRQ can't trigger before mmc_start_host()
is called.

The critical point here is that once sdhci_add_host() calls
mmc_add_host() -> mmc_start_host(), sdhci_add_host() is coded not to
fail. In other words, if there's a chance that mmc_start_host() may have
been called, and CD IRQs triggered, and the delayed_work scheduled,
sdhci_add_host() won't fail, and so cleanup is no longer via
sdhci_pltfm_free() (which doesn't free the IRQ or cancel the work queue)
but instead must be via sdhci_remove_host(), which calls mmc_remove_host()
-> mmc_stop_host(), which does free the IRQ and cancel the work queue.

b55a912aae mmc: sdhci-pxav3: set_uhs_signaling is initialized twice differently
commit b315376573 upstream.

.set_uhs_signaling field is currently initialised twice once to the
arch specific callback pxav3_set_uhs_signaling, and also to the generic
sdhci_set_uhs_signaling callback.

This means that uhs is currently broken for this platform currently, as pxav3
has some special constriants which means it can't use the generic callback.

This happened in
commit 96d7b78cfc ("mmc: sdhci: convert sdhci_set_uhs_signaling() into a library function")
commit a702c8abb2 ("mmc: host: split up sdhci-pxa, create sdhci-pxav3.c")'

Fix this and hopefully prevent it happening in the future by ensuring named
initialisers always follow the declaration order in the structure definition.
